Good find :)
The guy who started all this is called dr. Raed Arafat, a Palestinian, who is now under secretary of state for health and has been so for the last 5 years or so.
The British influence is quite obvious, I'd say :D

Interesting -- but I don't really see a British influence here. The nature of the work tends to mean that EMS organisations often have relatively flat management and supervisory structures. There are a lot of grades here but beyond the operational "provider" grades, I wonder if there are only a few posts in the supervisory/managerial grades?
The markings for the higher grades are fairly unusual but the hash marks under the caduceus for the paramedic grades look more like junior enlisted grades in the US Navy to me...!
Miklos, are there physicians in this grade structure or are all the managers from a paramedic background?

the ones with stripes and diamonds and cadduceus with wreaths are all physicians, the ones with chevrons and cadduceus without wreath are paramedics.

the caduceus would point to me more to a US influence of some sort as most other organisations uses the more appropriate rod of Asclepius.

the wings were added to show the speed with which they fly to the site ... basically, there are two medical emergency institutions: the "Ambulanta" which is is the old state EMS and the "SMURD", founded in 1991 by dr. Arafat, operating within the Fire Department (IGSU - General Inspectorate for Emergency Situations). The insignia are based on those of the latter, which are somewhat more British in appearence ... well, British with a healthy dose of Jordanian/Palestinian influence ...
